Q. Why was my registration not approved?

Your registration wasn't approved because of a few missing or incorrect details. Here's what you need to fix:

1. Documents must be in PDF: Upload your documents as PDFs.
2. Expired Physical: If your child's physical exam is out of date, you'll need to submit a new one.
3. Vaccinations Needed:
   • 7th Grade: First Meningococcal and Tdap booster shots.
   • 12th Grade: Second Meningococcal shot.
4. Use the Correct Physical Form: Make sure the physical is on the right form, available on the school's website under Parents/Community + Health Services + Required NYS School Health Exam Form
5. Emergency Medication Order: If your child has asthma requiring albuterol or an allergy requiring an epi-pen, please provide a doctor's order that states your child can self-carry the medication.
6. Incomplete Doctor Information:
   • The doctor must check "Family cardiac history reviewed."
   • The doctor must check that your child can "participate in all activities without restrictions."
7. Doctor's Signature and Stamp: The doctor needs to sign and stamp the physical form.

Q. I sent in a hard copy of my child's physical. Do I still need to upload it on Arbiter?

Yes, upload a copy of the most recent physical and immunization records with each registration. This is the fastest way to get your child approved.
