Working Papers

WORKING PAPERS INFORMATION
Working papers are required for everyone under 18 years of age for all types of employment.
Before working papers can be issued, please review the list of required documentation below and complete the attached forms. The completed documents need to be emailed to WORKINGPAPERS@manhassetschools.org.

1) AT-22 APPLICATION FOR EMPLOYMENT PERMIT

2) Copy of a Duly certified birth certificate or duly certified transcript or a record of baptism giving the date of birth, or if both are not available, a passport showing date of birth.

3) AT-16 PHYSICAL FITNESS CERTIFICATION
After receiving all of the documentation, Ms. Tabag will schedule a phone call with the student and review the information. Working papers will be issued upon satisfactorily complying with all requirements.
